1. Your name, address, and phone number
2. How long have you been homeschooling? 3. What other documentation of your homeschool education do you currently have?
4. Who was your primary instructor?
5. Are you prepared to enter into a graduation contract?
6. Do you understand that VHI will provide you with: A contract that contains the requirements set forth for VHI to issue you a diploma, A letter of recommendation for work or colleges/Universities, transcripts which detail your accomplishments, the ouline of a portfolio which you will complete this school year, and a diploma if all graduation requirements are met?
7. Do you understand that you are responsible for all homeschool record keeping for yourself?
8. Do you understand that VHI does not refund graduation contract fees if contract requirements are not completed by the student?
9. Please state "Graduation Option" if you are applying for the senior year graduation contract option.
After you submit this page you will find a link to the fees page to complete your Application process. Scroll down and choose Graduation Option to pay your fees. Payment of fees indicates agreement with the contract terms. Your Graduation Option Application will be reviewed and you will receive confirmation within 48 hours.
